1. Identify and prevent mental health issues that may arise again in the future.
2. Confirm the diagnosis of a mental health condition.
3. Identify and role out physical illness that may be causing or contributing to the patient’s symptoms.
4. Involve the patient when making decisions about his / her treatment plan.
5. Make changes in the patient’s treatment plan(if applicable).
